We welcome new and returning Capital Women’s Care patients. Our staff is committed to ensuring that the administrative aspects of your visit are handled with the same high level of professionalism and care as the medical portion.
Appointment Policy
Please arrive promptly and allow 20 minutes to update/complete the required paperwork. To help your visit go as smoothly as possible, please present the following at your appointment:
- Current insurance card(s)
- Driver’s license or legal photo ID
- Referral forms if necessary
New patients will be required to complete medical history, registration, and HIPAA forms; all patients will be required to complete the registration form.

Please print and complete the following forms at home and bring them with you to save some time on the day of your appointment. To view the forms, you may need to download Adobe Reader.
Accepted Insurance Plans
Capital Women’s Care participates with most major insurance plans. Please contact our office for more information about accepted insurance providers.
Emergencies
Obstetrics and gynecology often involves emergency procedures, surgical delays, and deliveries. We ask for your understanding, and if we are running behind schedule, our staff may attempt to notify you.
Cancellations
Prescription Refills
Feel free to phone in your prescription refill request to the office by calling 301-665-9098, option 3.
Prescription refill requests will be sent into your pharmacy twice daily, at 12 PM and 4pm. Prescription requests received after 4pm and on weekends will be completed the next business day. In order for us to call in your prescription refill, we will need the following information. Patient name, date of birth, medication name and dosage, pharmacy name and location, and a phone number with area code where we can reach you.
Authorization
Capital Women’s Care maintains the patient’s rights wherein all medical records are kept confidential according to all federal and state laws. Federal and state law requires the patient’s signature, or that of an authorized personal representative, to release or transfer medical records. The patient records are defined as all records that relate to the health of the patient and are prepared by or under supervision of a health care provider associated with Capital Women’s Care. Patients who provide records from another physician, outside of CWC, are encouraged to retain their own copy as Capital Women’s Care cannot provide a copy.
